Do you know a current and/or former patient of at least 12 years of age who you believe is a role model for others? If so, please nominate them today!
The Rainbow of Hope Award is an annual recognition of three Shriners Hospitals for Children — Twin Cities patients who best meet the criteria of a role model, which is defined as:
- Someone who has overcome physical limitations while maintaining a positive attitude
- Someone who serves as a mentor
- Someone who is a good example to their peers; or,
- Someone who has provided service to their community
A committee will review all nominations and select three award recipients. Due to the nature of the celebration, we request that nominations be limited to persons who are able to attend our Gala and Hospital Day. We also request that you notify the patient/family of your intent to nominate them prior to submission of the nomination form.
Please include photos, as well as any other supporting documents, such as news articles, that share the accomplishments of the nominee. Nominations may be emailed or mailed to the address listed below.
All nominations must be received by April 1, 2016.
Each winner is expected to be present at our first annual Gala on Fri., May 20 and our Hospital Day event held on Sat., May 21, 2016. Rainbow of Hope Award nomination form
